WebEpub 2016 Sep 9. OBJECTIVES To identify in vivo MRI markers that might correlate with cerebral microinfarcts (CMIs) on autopsy in patients with cerebral amyloid angiopathy (CAA). METHODS We included patients with neuropathologic evidence of CAA on autopsy and available antemortem brain MRI. WebInflammation with giant-cell reaction surrounding amyloid-laden vessels was identified in 7 of the 42 cases. The clinical symptoms in each of the seven were subacute cognitive decline or seizure rather than hemorrhagic stroke, the primary clinical presentation in 33 of 35 patients with noninflammatory CAA (p<0.001).

WebCONCLUSIONS These findings suggest that cerebral amyloid angiopathy-associated microbleeds and macrobleeds comprise distinct entities. Increased vessel wall thickness may predispose to formation of microbleeds relative to macrobleeds. At the molecular level, it is also closely related to Alzheimer's disease (AD) through the protein amyloid beta-peptide. There are currently no effective prevention ...

WebThe importance of heredity in the amyloid diseases has been recognized for many years [ 7 ]; some amyloid disorders appear to be entirely due to heritable abnormalities in precursor proteins and are inherited in an autosomal dominant fashion. In addition, the expression of acquired amyloidoses may also be affected by genetically determined factors. WebWe hypothesized that ICH caused by cerebral amyloid angiopathy (CAA) or hypertensive vasculopathy would have different distributions of MRI T2 white matter hyperintensity (WMH) and microbleeds. METHODS Data were analyzed from 133 consecutive patients with primary supratentorial ICH and adequate MRI sequences.

WebRESULTS The presence of vascular amyloid was a sensitive marker for CAA-related hemorrhage, occurring in all 28 specimens from brains with hemorrhage. Conversely, the appearance of fibrinoid necrosis in amyloid-laden vessels was relatively specific for CAA-related hemorrhage. WebFourteen autopsy cases with cerebral amyloid angiopathy (mean age at death 73 years, nine males) and three controls (mean age at death 91 years, one male) were included in the study. Intact formalin-fixed cerebral hemispheres were scanned on a 3 T MRI scanner.
